Ontario Patient Summary – HL7® FHIR® Implementation Guide (Release 1)
The Patient Summary document is a “health record extract comprising a standardized collection of clinical and contextual information (retrospective, concurrent, prospective) that provides a snapshot in time of a subject of care’s health information and healthcare.” (ISO/TR 12773-1:2009). It contains essential clinical information that enables a common view and understanding of the patient within the circle of care and may be used by health care providers to support:
- Transition and transfer of patients across the continuum of care.
- Coordination and collaboration around a patient’s care in the context of Ontario Health Teams.
- Safe provision of care in unexpected or unscheduled medical situations (e.g., emergency or urgent care)
The first release of the Patient Summary is limited in scope and currently includes:
- Medication Summary
- Allergies and Intolerances
- Problem List
- Immunizations
- History of Procedures
- Past History of Illness
Future scope of the Patient Summary will be shaped by Ontario stakeholders as well as Pan-Canadian alignment opportunities.
Scope
The Ontario Patient Summary Implementation Guide is developed based on HL7® FHIR® (Fast Healthcare Interoperability Resources) standard Release 4 and is intended to be used by implementers of digital health assets that support provision of clinical information at the point of care. It provides guidance on how to create and share patient summaries using FHIR and standard terminologies. It also describes the source information systems and interactions required to create, update, and share patient summaries between point of service systems and a provincial Patient Summary repository.
The guide is a web-based specification hosted on the Canadian FHIR Registry powered by SIMPLIFIER.NET*. The reader will be re-directed to the implementation guide’s home page on Canadian FHIR Registry website after registering and clicking the “Agree and Download” button.
Status
This implementation guide is a Balloted Draft for trial use and is subject to a formal review and approval process.
FHIR is an evolving trial-for-use standard, so additional changes by HL7 are expected. We will continue to monitor developments and assess whether there are impacts and publish updates as they emerge.
*SIMPLIFIER.NET is a web-based FHIR registry built by Firely that serves as the hosting platform for the Canadian FHIR Registry sponsored by Canada Health Infoway.
